Abstract

Minimally invasive gynecologic surgery has transformed the management of benign gynecologic conditions; however, evidence regarding its long-term clinical and patient-centered outcomes remains limited. This mixed-methods longitudinal study evaluated the extended gynecological, psychological, functional, aesthetic, and economic outcomes of women undergoing conventional laparoscopic or robotic-assisted minimally invasive surgery. Quantitative analyses demonstrated sustained reductions in pelvic pain, low late complication and recurrence rates, and stable improvements in functional recovery over long-term follow-up. Patient-reported outcomes revealed progressive enhancement of mental wellbeing, sexual function, and cosmetic satisfaction, with quality-of-life indices remaining consistently elevated beyond the early postoperative period. Robotic-assisted procedures were associated with slightly higher satisfaction scores and improved recovery stabilization, though overall outcome patterns were comparable between surgical modalities. Multidimensional outcome integration highlighted that clinical recovery, psychological adaptation, and patient satisfaction converge to define long-term procedural success. Economic analyses suggested that gains in recovery efficiency and reduced healthcare utilization may partially mitigate higher procedural costs associated with robotic surgery. Collectively, these findings indicate that minimally invasive gynecologic surgery provides durable, patient-centered benefits, with robotic-assisted approaches offering incremental advantages in selected domains. The results emphasize the importance of comprehensive long-term outcome assessment and support continued evaluation of emerging surgical technologies through standardized, high-quality research frameworks
